Use these checklists to prepare an initial notification. The insurer may request additional evidence after reviewing the circumstances and policy wording.

First actions

Protect people, property and evidence first.

Take reasonable steps to prevent further loss, report emergencies to the appropriate authorities, preserve damaged property where safe and notify us promptly. Do not admit liability or negotiate settlement with third parties without guidance.

Motor accident or theft

  • Completed insurer claim form when provided
  • Police abstract or report where applicable
  • Driver's licence and national ID
  • Vehicle logbook and insurance certificate
  • Photos, third-party details and incident description
  • Repair estimate or assessment instructions

Medical reimbursement

  • Completed claim or reimbursement form
  • Itemised invoices and official receipts
  • Diagnosis, prescriptions and clinical notes
  • Member card or policy details
  • Referral or pre-authorisation where required

Fire, burglary or property damage

  • Incident and discovery timeline
  • Police or fire-brigade report where applicable
  • Photos or video before clean-up where safe
  • Asset, stock and purchase records
  • Repair or replacement estimates
  • Security or alarm records for theft claims

Liability claim

  • Third-party demand, summons or correspondence
  • Incident report and witness details
  • Contracts, work orders or service records
  • Photos, CCTV or other preserved evidence
  • Do not admit liability or promise payment

Life or funeral benefit

  • Completed claimant statement
  • Certified death certificate
  • National IDs for deceased and claimant
  • Beneficiary or relationship evidence
  • Medical or police documentation where requested
  • Bank details through the insurer's approved process

Work injury

  • Employer incident report
  • Applicable statutory forms
  • Employee ID and payroll information
  • Medical reports, invoices and sick-off records
  • Witness and workplace investigation details
  • Authority notification where required

Travel incident

  • Policy certificate and travel itinerary
  • Carrier or police report for loss
  • Medical reports and itemised invoices

Marine or goods in transit

  • Commercial invoice and packing list
  • Bill of lading, airway bill or delivery note
